Looks like the filter was misaligned when the cover was fitted. Not mine. Though that was the reason the engine failed in the car, before I bought it.
Had been fitted for approx 4-5k miles, engine gave up while towing. Pistons have overheated, and started melting...

Thanks Mikey for taking the time to show that..... It makes you wonder how many of the turned bearing failures are all caused by that issue.... It is quite reassuring if we believe this is the major cause....

For those with very limited engine knowledge (eg me) could you explain what happens because the oil filters been incorrectly fitted? I'm interested

Oil from the oil pump is directed to flow through the filter and then on to the oil passages in the engine maintaining the correct oil pressure to ensure bearings and tappers operate correctly, if the said fliter is not engauged correctly on the sealing O rings of the lower tab shown, not only does the oil not reach the engine parts at the correct pressure ( oil pump is able to mask this from bringing on the oil light warning) the filter as shown will get crushed when the top cover is fitted cause further distortion amplifying the oil flow issues.
So as our Guru mechanic has clearly shown, double check before lobbing on the top!!!

Mikey, was this one a genuine OEM filter? Have you seen any pattern in what makes of filters have been used, or is this purely down to human-error?

This is human error the filter was not fitter correctly, the little tab shown pointing up in the last picture should have been located in the feed hole on the block, it was not hence the whole filter has been squashed.
