Diesel in water

Just topped my coolant up and realised there is diesel floating on the top. Anyone have a clue why this could be

England 2005 Discovery 3 TDV6 HSE Auto Rimini RedDiscovery 3

There are 2 diesel to coolant heat exchangers.

One under the car near the starter. This has higher pressure I think

And one in the Vee of the engine where the leak off pipe connects
