Belt tensioner
Thought I could hear a new noise. Like a failing bearing, engine speed related. I kept imagining it was the gearbox input shaft bearing, but I decided to start looking somewhere easier. Took the front end drive belt off to check the alternator, water pump and tensioner/idlers, and guess what I found!

Lucky escape, would have been nasty hurtling down the motorway and it letting go.
