V8 pedal and throttle body seem out of sync

Having just done front control arms and restored some handling I decided to resume the quest from a few years ago to try and solve blunted acceleration at low revs...

Never got to the bottom of it, no error codes, engine lights, or apparent vacuum leaks. Has never got better or worse. Flies as normal 2500ish revs or above so not a big issue, but acceleration from rest isn't as perky as it was originally and would be fun to have it back at full power for surprising other cars at the lights.

On a cold start I noticed that the engine feels a bit lumpy if you rev it before the revs settle down (no misfire codes), and even once they do settle revs are smooth but very sluggish to rise even if giving the pedal a good stomp. Live data showed the pedal positon data changing in accordance with the pedal movement but the throttle body position really lagging. Does that give any clue, e.g. pedal fine, throttle body faulty, or a sensor of some sort, or not really that informative? Not very mechanically minded here just looking to see if I can point mechanic in the right direction or make an educated guess before I start firing the parts canon at it. Cleaning the throttle body was the first thing I tried back at the beginning and I'm sure mechanic checked MAF back then too.

Just as investigation, if you change to sand mode, does the bahviour change at all? It should become more responsive and "aggresive" than in normal mode with the same pedal travel.

Just tried it in sand mode (cold start) but no discernible difference still lumpy then laggy and blunted. Though I just sat and revved it, will see if I can feel any difference on the move when I head out later...
